Are You a Carer?

A carer is someone, of any age, who provides unpaid support to family or friends who could not manage without this help. This could be caring for a relative, partner or friend who is ill, frail, disabled or has mental health or substance misuse problems. Anyone can become a carer, in most cases becoming a carer is not out of choice, it just happens.

It is important that carers are supported to enable them to continue to provide care for a relative or friend.

Register as a Carer

If you are a patient of the surgery and a carer, you should register with us so that we know. This can help as we may be able to offer support and help accessing appointments etc. We can also offer you a referral to the local carer information services who can help and support you further should you wish it. To register, either call in at either surgery and request a Carer Registration Form, or download and print a copy of the registration form below and hand it in at your surgery.
