If you live within the boundary of the Neyland and Johnston Medical Practice, you must complete the Family Doctor Services Registration Form (GMS1W) along with our Patient Information Questionnaire. Both forms are available from the Neyland Surgery or Johnston Surgery) or by downloading and printing from our website. You should complete the form and return it in person to either surgery along with photograph identification and proof of your new address (if available). To complete the form, you will need your NHS Number, which is available from your current GP, or on the right-hand side of your prescription.
If you are on repeat medication, it is advisable to order at least a 4-week supply of your medication (ideally an 8-week supply) from your current GP. Please attach a copy of your repeat medication slip (right-hand side of your prescription) to your Patient Information Questionnaire, to avoid delays in issuing a repeat prescription.
